Makes you wonder how bad things are getting in terms of H5 prevalence when they have to do announcements informing people of its symptoms through the same channels as they announce seasonal flu symptoms. Needless to say that this thing is getting much better at transmission.

DJ, H5N1 -without symptoms-in pigs/swine may be very alarming...However very likely only a small number of cases of human H5N1 are detected..5%-10%...so around 50 confirmed cases may stand for 500 to a 1,000 "real" human cases...And -DJ-some of it may be in Mexico...

The fall bird migration season is well underway, and Canada - after going more than 6 months without reporting any outbreaks of H5N1 in poultry - has reported a steep rise in outbreaks since the third week of October.  

In last 10 days of the month, the CFIA reported 11 outbreaks

Over the first 8 days of November, the CFIA has added 10 more outbreaks.

Meanwhile, in the United States, the USDA is also reporting 21 outbreaks in poultry over the past 30 days.
Since the first of November, the USDA has reported new outbreaks from 5 different western states
According to a brief statement (below) from the USDA, we know that the Oregon poultry & swine outbreak of H5N1 was due to genotype D1.2while last month we learned that the Washington state outbreak (and spillover into humans) was from genotype D1.1. 

Additionally poultry outbreaks in California, along with the recent outbreak in poultry in Utahhave been identified as from the bovine B3.13 genotype, which is primarily associated with cattle. 

The genotype affecting Canada's poultry this far has not been announced, however the CFIA website states:  To date, the genotype B3.13 found in U.S. dairy cattle has not been detected in birds or any other animals in Canada.

Thus far, more than 100 genotypes of the H5N1 clade 2.3.4.4b virus have been identified in North America, and scores of other genotypes are circulating around the globe. 

This week, South Korea reported an HPAI H5N3 subtype in wild birds for the first time, and the UK reported more than 2 dozen dead wild birds from HPAI H5N5, and an outbreak of HPAI H5N5 at a poultry farm in Yorkshire. 

A reminder that the HPAI H5 family of viruses continues to expand and diversify, and that we face multiple credible threats here, not just H5N1 in dairy cows. 

The amount of HPAI H5 in wild birds - and in the environment - is expected to rise as we head deeper into the fall and winter, and all flock owners (large and small) need to increase their biosecurity (see USDA's Defend the Flock).

And while the risks are believed small, the CDC also offers the following advice to pet owners on avian flu..

Earlier today I blogged on the recent uptick in H5N1 outbreaks in poultry in British Columbia (see Canada: B.C. Reports 10 More Poultry Farm Outbreaks Of HPAI H5), and its growing prevalence in the environment.

This evening (h/t Treyfish on FluTrackers) the British Columbia Government has announced a presumptive positive H5 case in a teenager, hospitalized at B.C. Children's Hospital.  

Details are scant at this time - and there is no mention of contact with poultry, or the patient's condition - in this initial news release.  Hopefully we'll learn more tomorrow.

The individual is a teenager from the Fraser Health region who is receiving care at BC Children’s Hospital. A public-health investigation has been initiated to determine the source of exposure and identify any contacts.

Public health is following up with contacts who may have been exposed in order to assess for symptoms and provide guidance on testing and prevention measures. There have been no further cases identified at this time. The source of exposure is very likely to be an animal or bird, and is being investigated by B.C.’s chief veterinarian and public health teams.

Start of flu-season is increasing further risk of dangerous types of flu...At least two pigs also tested positive for H5N1...Pigs can catch all four types of (known) flu (A,B,C, and D) -humans so far only A and B-type of flu...H5N1 may spread WITHOUT !!! symptoms in pigs...so very likely more H5N1-infections in pigs...The two detected cases were from a mixed farm with also cattle and/or poultry-infecting the pigs...

While it may not tell us where HPAI H5 will go in the days and weeks ahead, the events of the past 21 days have demonstrated how quickly the status quo with avian flu can change.  

On October 21st, the CDC counted 27 human H5 infections in the United States in 2024. 

Three weeks later that number is officially 46 (a gain of 19), but this doesn't include 4 cases confirmed by states (California & Washington) but not the CDC, a 2nd (strongly suspected) case in Missouri, or any of the 8 cases turned up last week by retrospective serology tests. 

Three weeks ago, both the United States and Canada had gone months without substantial outbreaks in poultry, but between them have since reported > 40 outbreaks. 

At the same time the known number of  American dairy herds infected by H5N1 has increased by 46% (from 324 to 473), across (now) 15 states.

All of that by itself would be concerning, but this doesn't begin to scratch the surface of H5 related events of the past 3 weeks. With that in mind, a mini review of 10 other H5 avian flu related events or revelations from the past 21 days.

  • On October 24th, we got the long awaited Serology Results On Missouri H5 Patient's Contacts, which found no evidence of transmission to HCWs, but did strongly suggest a housemate was infected at the same time as the index case.  The source of these infections has yet to be identified. 


This is not a comprehensive list, and some honorable mentions over the past 3 weeks include:
Whether past is prologue remains to be seen - but based on the events of the past few weeks - we could be in for a bumpy ride.

Over the past 6 months reporting of novel flu cases from Mainland China has dropped appreciably, with the last H5N6 case reported in July, and only 4 H9N2 cases reported between May and the end of October .

Of course, a lack of reports doesn't always mean a lack of cases, since China often only belatedly reports novel flu cases.  Sometimes this is because of delays in testing, and sometimes because it is politically expedient.

Today, Hong Kong's CHP has published a bare bones listing of 7 newly announced H9N2 cases, all of which occurred in just 3 weeks in Mainland China. 

While LPAI H9N2 is admittedly not at the very top of our list of pandemic concerns, the CDC has 2 different lineages (A(H9N2) G1 and A(H9N2) Y280) on their short list of influenza viruses with zoonotic potential (see CDC IRAT SCORE), and several candidate vaccines have been developed.

Only about 140 cases have been officially reported over the past 20 years (see FluTrackers list), and most (but not all) of them have reported mild or moderate illnessSeroprevalence studies, however, suggest the infection is far more common than we believe. 

H9N2 is poorly controlled in Chinese poultry, despite the use of vaccines (see J. Virus Erad.: Ineffective Control Of LPAI H9N2 By Inactivated Poultry Vaccines - China), which has led to the creation and spread of numerous of genotypes.

H9N2 also reassorts with, and often enhances, other novel influenza viruses (including H7N9, H5N1, and H5N6), making it an important co-conspirator (see Vet. Sci.: The Multifaceted Zoonotic Risk of H9N2 Avian Influenza).

Details are scant, and the significant of today's announcement is difficult to determine.  It could be due to increased surveillance, or it might signify an increase in transmission.

Late on Saturday we learned that Canada had identified their first locally acquired human infection with HPAI H5 in a teenager from the Fraser Valley region.  The patient was reported to be hospitalized in the B.C. Children's Hospital, and was in intensive care. 

Today the B.C. provincial health officer - Dr. Bonnie Henry - held a lengthy news conference, and fielded questions from reporters, on the condition of this teenager, and their investigation. 

Unlike the 45+ American cases who have reported mild illness, this case is listed in critical condition and is suffering from ARDS (Acute Respiratory Distress Syndrome).  The patient - reportedly a healthy teenager - fell ill on November 2nd, with symptoms of conjunctivitis, fever and cough and was seen at a local emergency room, and sent home.

Over the next few days the patient's condition worsen, and they were admitted to B.C.'s Children's Hospital on Friday night (Nov 8th), and a presumptive test for H5 influenza came back on Saturday.  The patient was given antivirals and admitted to the I.C.U.

B.C. health officials have contacted 35-40 close contacts, and have offered them antivirals and will follow up with them for 10 days.  So far, none have shown signs of illness. 

While the investigation is ongoing, so far they've been unable to determine how this teenager was exposed.  The patient did not live on, or visit, a farm or have contact with poultry, or wild birds.  Nor did any of their known contacts. 

The patient did have contact with dogs, cats, and a reptile, but none have tested positive for influenza.  One of the dogs was reportedly symptomatic, but follow up testing has turned up nothing of value. 

While they intend to keep looking for a plausible source - as we saw with the Missouri case last August - answers may never be found. 

Complete sub-typing, and genetic sequencing will take a few days.  For now, H5N1 is strongly suspected, but earlier today WOAH reported at least one outbreak of HPAI H5N2 in a B.C. poultry farm. 

The entire press conference (about 40 minutes) is worth watching. 

While Dr. Henry continues to call this a rare event, it is obvious she takes this seriously, and she warns people to avoid contact with wild birds or sick animals, and to keep their pets away from possible exposures. 

One of the big concerns that Dr. Henry addressed is H5N1's historical disproportional impact on children, adolescents, and younger adults.  This is a topic we've addressed often, most recently last week in Preprint: Immune History Shapes Human Antibody Responses to H5N1 Influenza viruses.

While many have been reassured by the mild presentation of H5N1 in the United States over the past 6 months, this is a reminder that an H5 infection isn't always benign, and that we need to take this threat seriously.


DJ, catching flu may offer immunity protection to (other types of) flu. So a young person/host may not have immunity after catching flu yet.

🚨This mutation in NA of human #H5N1 is well known to assoc w/ drug resistance to oseltamivir. It should be a clear trigger to immediately diversify SNS holdings. Fewer than 1M doses of baloxavir held, w/ 12+ mo lead to make more. Did mutation appear pre or post oseltamivir treatment? Is this NA mutation also in #H5N1 from pigs & from human case in Canada? Glad baloxavir & IV antivirals were available for Canada, timing is a factor on their effectiveness. Urgently need to ⬆️ surveillance, ⬆️ SNS drug diversity, ⬆️ data for combo therapy, & ⬇️ data sharing lag

With their latest update, the USDA's count of H5 infected herds in the country now sits at 505, across 15 states, with the addition of 13 new herds from California (n=291).  

Of course, with no national mandatory testing of bulk milk or dairy herds, we don't really know how many dairy herds have been affected. 

This `Don't test, don't tellpolicy extends beyond just cattle.  Despite sporadic reports of HPAI H5 in goats, in alpacas, and most recently in swine, there appears to be little appetite for testing those herds either.

And state submissions to the USDA's Detection of HPAI in Mammals list have ground to a halt over the past few months, with only 2 samples shown as collected since late July.

DJ-I continue this story under "new pandemic" -latest news...
At least the US is testing/reporting some cases...NOT in time-missing hundreds of cases...
CoViD will be opening up for H5N1...soon no longer mild...(Canada case as indication).
