ABSTRACT

The novel H7N9 avian influenza virus (AIV) was demonstrated to cause severe human respiratory infections in China. Here, we examined poultry specimens from live bird markets linked to human H7N9 infection in Hangzhou, China. Metagenomic sequencing revealed mixed subtypes (H5, H7, H9, N1, N2, and N9). Subsequently, AIV subtypes H5N9, H7N9, and H9N2 were isolated. Evolutionary analysis showed that the hemagglutinin gene of the novel H5N9 virus originated from A/Muscovy duck/Vietnam/LBM227/2012 (H5N1), which belongs to clade 2.3.2.1. The neuraminidase gene of the novel H5N9 virus originated from human-infective A/Hangzhou/1/2013 (H7N9). The six internal genes were similar to those of other H5N1, H7N9, and H9N2 virus strains. The virus harbored the PQRERRRKR/GL motif characteristic of highly pathogenic AIVs at the HA cleavage site. Receptor-binding experiments demonstrated that the virus binds α-2,3 sialic acid but not α-2,6 sialic acid. Identically, pathogenicity experiments also showed that the virus caused low mortality rates in mice. This newly isolated H5N9 virus is a highly pathogenic reassortant virus originating from H5N1, H7N9, and H9N2 subtypes. Live bird markets represent a potential transmission risk to public health and the poultry industry.

Today talking to farmers, they used to pay $3.50 per chicken and now is like $15 and actually they don’t get all the chickens they request…so the price of eggs and chicken is going up big time….milk too….the bird flu is real and will get worse if we don’t take it seriously…eggs will reach 1$ per egg..😔

When infectious diseases are ignored or neglected—as H5N1 has been—the consequences extend far beyond public health. They can cause severe economic disruption and food insecurity/shortages.

As H5N1 avian influenza continues to spread in wild birds and spills over into domestic birds and mammals of many kinds, we’re becoming more aware of the risks to domestic mammals and there are more questions about test selection and interpretation. Fortunately, testing for H5N1 influenza is relatively straightforward.

  • For cats and dogs, the recommended samples for testing are typically oropharyngeal swabs, plus or minus nasal swabs (plus or minus other tissue samples of the animal is deceased. 

What tests are currently being run by veterinary diagnostic labs?

PCR is the most accessible and useful test; diagnostic labs basically offer two types of influenza PCR: matrix / pan-influenza A PCR, and strain-specific PCRs (see below). Different commercial labs offer different tests or combinations of these tests, and the tests offered may also vary by species (i.e. what’s routinely done for dogs can be different from cats). Test offerings may also change over time as labs adapt to the ever-changing situation with flu in different populations. If influenza is a consideration in your patient, check with your lab regarding which tests they will run (particularly if you are submitting samples for a respiratory PCR panel) and how to interpret the results, including whether or not the test will detect H5N1 flu if that’s a concern.

Take home message

  • Talk to your lab to know what they can (and can’t) do in terms of testing for H5N1 flu, and follow up testing.
  • We can’t just stop at “flu positive.” Any such result needs to be scrutinized to make sure we know the strain (or strains) involved.

Coughing, fever, a lame child. The flu is rampant. What if you get a call from childcare or school to pick up your child? What are your rights and obligations as parents? Employment lawyer Pascal Besselink of DAS knows what to do.

Childcare and school. Influenza. A child has to go home. As a parent, can I drop everything out of my hands?

Besselink: "Employment law has various regulations. For example, emergency leave; having to leave urgently because you have to pick up your child from daycare or school and there is no one else who can do that. You can also fall back on this if, for example, the neighbor calls who says you have a fire in the house or a burst water pipe. This is a short period of time, to be determined reasonably."

Then you have picked up your child, let's call him Daan, and that takes a little longer than expected. Daan is not better yet. Can I get longer leave?

"Then we have short-term care leave. You can take this if, for example, grandpa and grandma are not available to cover the childcare at your home. This can be done for a maximum of 2 weeks. During that period, you are entitled to 70 percent wages, capped at 70 percent of the maximum daily wage. The employer can refuse a request if there are compelling business interests and also request information from you about the necessity. For example: isn't there someone else who can take care of Daan. And then there is also long-term care leave. It is a maximum of 6 weeks, but unpaid."

What most often occurs is a compromise between employer and employee. How does that work?

"You indicate to your manager that something is going on at home and that you cannot be missed there. You don't know exactly how long it will take and the employer says 'I hope you'll be back tomorrow' and takes on part of the costs of your absence and you take some vacation days, for example. Something like that is then done in consultation."

Does it matter whether you are the father or the mother?

"No, it doesn't. A couple agrees among themselves who will pick up Daan. A man may work more hours, but if the partner is also at work at that time and he can leave, the father will pick him up. One of the two will then have to arrange this with his or her employer."

Anything else?

"So there are arrangements, but in practice a lot is done in consultation. It's not rocket science."

The annual flu epidemic has started in the Netherlands, the RIVM and Nivel said on Wednesday. And that is visible in the increasingly crowded hospitals and at schools where teachers are struggling to fill rosters and cover for sick colleagues.

While our biggest avian flu concern remains HPAI H5N1, over the years we've seen closely related H5N2, H5N3, H5N4, H5N5H5N6, H5N8, and H5N9 viruses - the product of H5N1 reassorting with other LPAI viruses - infecting both birds and mammals around the globe.

While most have struggled to compete with H5N1, between 2014-2020, HPAI H5N8 became the dominant subtype globally, and HPAI H5N6 viruses have shown persistence in China and East Asia for more than a decade. 

Since regaining its mantle in 2021, H5N1 has had little competition. But over the past few years we've seen reports of an H5N5 variant - first in Europe and Asia, then in Canada - which has not only managed to survive, it appears to be thriving.

Last July, in Cell Reports: Multiple Transatlantic Incursions of HPAI clade 2.3.4.4b A(H5N5) Virus into North America and Spillover to Mammals, researchers reported finding the mammalian adaptive E627K mutation in a number of samples.

Thus, while A(H5N5) viruses are comparably uncommon, their high virulence and mortality potential demand global surveillance and further studies to untangle the molecular markers influencing virulence, transmission, adaptability, and host susceptibility.
Not quite 3 weeks ago, the Icelandic Food and Veterinary Authority (MAST) reported an H5N5 Infection In Domestic Cat.

While I'm not expecting a shifting away from H5N1 anytime soon, it isn't beyond the realm of possibility that a new subtype - H5N5, H5N2, H5N6 or some other n-type - could emerge and give it a run for its money. 

As a segmented virus with 8 largely interchangeable parts, the flu virus is like a viral LEGO (TM) set which allows for the creation of new subtypes or genotypes. Each gene segment can host multiple unique amino acid changes (e.g. HA-Q226L, PB2-E627K, etc) that can alter the behavior - and host range - of the virus.
All reasons why we shouldn't be surprised when the flu does the unexpected.

Emerg. Microbes & Infections: Emergence of a Novel Reassortant HPAI Clade 2.3.4.4b A(H5N2) Virus, 2024


The superpower of influenza A viruses is their ability for different subtypes to co-infect a host, swap genetic material, and generate a new `hybrid' virus; a reassortant.  This reassortment can generate new genotypes, or - if the HA and NA are involved - a new subtype. 

Reassortment - or Antigenic`Shift' - can produce abrupt changes in the behavior of the generated hybrid.  Most are evolutionary failures, unable to compete with its parental strains. But every once in awhile, a new, `improved', virus emerge.  

As any virologist will tell you; `Shift Happens'

Reassortment is the process that enabled an avian H5N1 virus (B3.13) to infect cattle, and has recently produced a more virulent D1.1 genotype, which is spreading rapidly across the nation.  It produced a new H5N9 subtype in California poultry, and it could potentially lead to a hybrid of seasonal and avian flu sometime in the future (see Preprint: Intelligent Prediction & Biological Validation of the High Reassortment Potential of Avian H5N1 and Human H3N2 Influenza Viruses).

Today we have a report, published this week in Emerging Microbes & Infections, of the discovery of an HPAI H5N2 clade 2.3.4.4b virus in Egypt, one that came about through the merger of the H5N1 virus and the ubiquitous, and highly mutable, LPAI H9N2 virus. 

We've often discussed the role that H9N2 has played in the creation of some of the most dangerous HPAI viruses, including H5N1, H5N6, H10N3, and H7N9 (see Transboundary & Emerging Dis.: The H5N6 Virus Containing Internal Genes From H9N2 Exhibits Enhanced Pathogenicity and Transmissibility).

While much as been made over the fact that humans may have some limited cross-immunity to the N1 component of H5N1, a new subtype - sporting an N2 or an N9 surface gene - might prove far more problematic.

While the world seems to be focused on HPAI in North America, this should remind us that H5Nx viruses are occurring all around the globe, and that a new threat could emerge anytime, and from just about anywhere.

I’m not commenting on every new report of H5N1spillover into a domestic animal because it’s not really news anymore. That doesn’t mean it’s not concerning. It definitely is. However, we expect these spillovers to continue and were going to see continued cases as long as this virus is circulating in wild birds, or, in the US, in dairy cattle.

Despite that, a recent case in Oregon highlights something important. Most reported infections in cats have been severe disease, usually with neurological involvement. We’ve been unclear whether this is because cats typically get severe neurological disease when they are infected or whether we tend to only test cats with severe disease. The fraction of milder disease is unknown and that’s really important for considering testing, either of clinical cases or surveillance testing.

Should we consider any outdoor/raw fed cat with any signs of respiratory disease flu suspects? Maybe. It’s certainly possible that H5N1 can also cause typical flu-like disease/upper respiratory infection. So, I’d recommend not completely discounting it in any case, but paying particular attention the more severe the disease is.  

As with most emerging diseases, it’s a fluid environment and it’s tough to figure out where to draw the line, balancing protection and practicality. As we learn more, that line will likely move so we must keep an eye on new developments but take reasonable measures in the interim. Personally, I’m always a ‘test more’ and ‘err on the side of being more aggressive at the start and then de-escalate’ but there’s also a practical limit in how far we can go.

DJ, It becomes more and more likely each day H5(N1 etc) is mixing with "human" flu types...If not in a human in then in a cat, pig, dog, mink, ferret....

So we are getting closer to a "high(er) risk type of flu" spreading...It may be in the US but might be (also) happening in Vietnam, China, Africa or Europe...

It is a matter of definition/choices (by the WHO) to go for "pandemic" or Public Health Emergency of International Concern" (PHE-IC).
