Abstract

Background: In early April 2024 we studied two Texas dairy farms which had suffered incursions of H5N1 highly pathogenic avian influenza virus (HPAIV) the previous month. Methods: We employed molecular assays, cell and egg culture, Sanger and next generation sequencing to isolate and characterize viruses from multiple farm specimens (cow nasal swab, milk specimens, fecal slurry, and a dead bird).

Results: We detected H5N1 HPAIV in 64% (9/14) of milk specimens, 2.6% (1/39) of cattle nasal swab specimens, and none of 17 cattle worker nasopharyngeal swab specimens.

We cultured and characterized virus from eight H5N1-positive specimens. Sanger and next-generation sequencing revealed the viruses were closely related into other recent Texas epizootic H5N1 strains of clade 2.3.4.4b. Our isolates had multiple mutations associated with increased spillover potential.

Surprisingly, we detected SARS-CoV-2 in a nasal swab from a sick cow.

Additionally, 14.3% (2/14) of the farm workers who donated sera were recently symptomatic and had elevated neutralizing antibodies against a related H5N1 strain.

 Conclusions: While our sampling was limited, these data offer additional insight into the large H5N1 HPAIV epizootic which thus far has impacted at least 96 cattle farms in twelve US states.

 Due to fears that research might damage dairy businesses, studies like this one have been few. We need to find ways to work with dairy farms in collecting more comprehensive epidemiological data that are necessary for the design of future interventions against H5N1 HPAIV on cattle farms.

DJ...So with very limited testing a cow tested positive for both CoViD and H5N1 !!!!!
The main reason why there were not more co-infections/Flu-Rona detected is lack of testing...

In January 2022,  https://en.wikipedia.org/wiki/Israel - Israel  reported, for the first time, a mixture of COVID-19 and influenza infections, colloquially known as "flurona". https://en.wikipedia.org/wiki/Twindemic#cite_note-IND-20220101-9 - [9] https://en.wikipedia.org/wiki/Twindemic#cite_note-DB-20220103bln-10 - [10] https://en.wikipedia.org/wiki/Twindemic#cite_note-WP-20220105-11 - [11] https://en.wikipedia.org/wiki/Twindemic#cite_note-NYT-20220107-12 - [12]  In  https://en.wikipedia.org/wiki/Brazil - Brazil , four cases of the double infection have been identified, including a 16-year-old male from  https://en.wikipedia.org/wiki/Rio_de_Janeiro_%28state%29 - Rio de Janeiro . In  https://en.wikipedia.org/wiki/Fortaleza - Fortaleza  of  https://en.wikipedia.org/wiki/Cear%C3%A1 - Ceará  state, two children, including a one-year-old child tested positive without complications, and also a 52-year-old man who did not need hospitalization. https://en.wikipedia.org/wiki/Twindemic#cite_note-13 - [13]  In  https://en.wikipedia.org/wiki/S%C3%A3o_Paulo_%28state%29 - São Paulo , the Secretariat of Health announced that its state had 110 cases in 2021. https://en.wikipedia.org/wiki/Twindemic#cite_note-14 - [14]

Flurona infections have also been reported in the United States, the  https://en.wikipedia.org/wiki/Philippines - Philippines ,  https://en.wikipedia.org/wiki/Hungary - Hungary  and  https://en.wikipedia.org/wiki/Romania - Romania . https://en.wikipedia.org/wiki/Twindemic#cite_note-WP-20220105-11 - [11]

DJ, the main problem-if there is even any testing in most cases testing stops after detection of-in most cases-CoViD...Why look further for any kind of flu ?

So very likely "flu-rona" is very under reported!

The limited data may not provide much info on increased risks. But in general both "flu" and "CoViD" itself are seen as a serious healthrisk-so a combination of both is "serious"...

https://health.clevelandclinic.org/what-is-flurona - https://health.clevelandclinic.org/what-is-flurona or https://health.clevelandclinic.org/what-is-flurona ;

Flurona isn’t a proper medical diagnosis, but it has become an increasingly popular term for what happens when you get infected by both  https://health.clevelandclinic.org/flu-vs-covid-19-can-you-tell-the-difference/ - the flu and COVID-19  at the same time. That said, the term flurona, though catchy, is kind of misleading. The experience of getting more than one infection at the same time is not isolated to just COVID-19 and the flu. You can get co-infections of multiple respiratory viruses at the same time — meaning you can get infected with any combination of COVID-19, the flu, RSV,  https://my.clevelandclinic.org/health/diseases/12342-common-cold - rhinovirus https://my.clevelandclinic.org/health/diseases/23022-adenovirus - adenovirus  and other viruses.
-

In the case of co-infections, it’s possible for some of these symptoms to be more severe. For example, you could experience a higher fever or prolonged fatigue. But knowing exactly how co-infections will affect you is difficult because everybody reacts to multiple viruses in different ways.

For severe infections, Dr. Rhoads says there are oral therapies available to help treat both COVID-19 and the flu. These therapies fall along the lines of  https://my.clevelandclinic.org/health/drugs/18887-oseltamivir-oral-suspension - oseltamivir (Tamiflu®)  and  https://health.clevelandclinic.org/covid-19-pill-treatment/ - antiviral pills .

DJ, I think it is realistic to claim "we do not know" what the risks may be for "Flu-Rona" since only a very limited number of (human) cases have been detected...

CoViD is known to result in damaging a.o. the immune system so other diseases get more chances.

Masks do offer protection-also glasses/a face-shield can stop infection via the eyes...

https://www.researchgate.net/figure/Case-fatality-ratios-COVID-19-versus-US-Seasonal-Flu-19_fig3_346440440 - https://www.researchgate.net/figure/Case-fatality-ratios-COVID-19-versus-US-Seasonal-Flu-19_fig3_346440440 or https://www.researchgate.net/figure/Case-fatality-ratios-COVID-19-versus-US-Seasonal-Flu-19_fig3_346440440 ;

comparison has been done between the CFR during the outbreak of COVID-19 in China in 2019-2020 with CFR of the US seasonal flu in 2018-19 ( Figure 5). The CFR of seasonal flu in the US is around 0.1% to 0.2%, while the CFR for COVID-19 measured in the study was 2.3%. ...

DJ, So CoViD has a higher case fatality rate (CFR) than Flu...but it may be to easy to add the CFR's of both...Very likely-in theory-the CFR for FluRona will be HIGHER than CoViD CFR and FLU CFR...because the combination may result in more severe disease and risk for complications.

(But I am NOT an expert-just try to make some sense !)
