As outbreak gets worse, transparency & timeliness will get worse. Politics will play bigger role as we near election.

DJ, My view; H5N1 going human-to-human does NOT fit in a (E)CDC "scenario/script"...it is "outside the box"...NOT supposed to be happening...
When you follow the H5N1 "story" than you know H5N1 must have been widespread both in birds and mammals.
In cattle part of the spread very likely is airborne (droplet-6 feet/2 meter distance or aerosol-wide area).
So it has to result in human cases-very likely spreading via droplet/aerosol to other humans...
For now "mild symptomatic" cases were reported-as far as tested. In the US no one has died -yet- from H5N1...as far as we know.
However the number of H5N1 human cases in the US -most farmworkers- may run in the hundreds...
Again-I am NOT any kind of expert !
Since infected cattle a.o. got exported to Mexico (from Texas a.o.) and raw milk consumption in Mexico is higher than in the US H5N1 may be in Mexico as well...but for now also a/mild-symptomatic.
A lot of farm workers are from Central America-if ill they may be deported back to that -kept poor-region...

DJ, So "twitter experts" (to put it that way) alarm about CDC NOT reacting to possible symptomatic H5N1 cases a.o. in Oregon. The CDC stating recent testing in Michigan farmworkers did NOT detect antibodies for H5N1 ?
Maybe the level of testing is too small ? The wrong groups ?

A. Very likely H5N1 is spreading human-to-human in the US
B. The CDC or other US agency has to make such a "story" official
C. In this early stage it may not be that easy to "get a finger on it"...because H2H may still be limited. The farm workers most at risk may be illigal immigrants not willing to cooperate...
Stories of farmworkers getting infected while working with infected cattle (eye infection during milking a.o.) are several MONTHS old...Testing is on a voluntary basis...

DJ, This is the official CDC confirmed cases list of H5N1 human spread in the US. So 10 cases confirmed in 2024-7 of them in Colorado. The last 6 cases-all from Colorado-in poultry workers...the other 4 cattle infected (2 from Michigan, Texas and Colorado each 1)
Also spread in mammals-detection goes on-worldwide.
SE Asia also did see several H5N1 cases this year-all linked to birds.
For that matter the US may detect bird-mammal-human trajects. It may be that -on a limited scale-mammal-human infections also happen in other places.
For the US it is likely mammal-human spread has been going on for over 3 months ? (Texas case reported april 1).
The H5N1 virus did get enough time to mutate to humans...

We continue to get more reassurance than details from federal agencies, although it is unclear how much of that is due to difficulties getting information and/or cooperation from local entities.
Genetic sequences continue to be deposited belatedly, and are often missing critical meta-data (see https://gisaid.org/resources/gisaid-in-the-news/highly-pathogenic-avian-influenza-outbreak-in-the-united-states/ - GISAID comment below), making independent analysis difficult.
This collection includes many virus sequences from dairy cows, but also closely related viruses detected in poultry and wild birds and in mice, cats and other mammals as well as the recent human infections. Although metadata such as sampling date and location are unfortunately missing from recent datasets, the available data allow a close watch on mutations that may arise as a consequence of virus adaptation to new hosts.
Unknown is how the bovine B3.13 genotype has managed to spillover into poultry, and whether it is now spreading via wild birds.
Other than reporting that PPE use by poultry workers was `not optimal' (blamed on excessive heat and fans inside barns), we've seen little in the way of epidemiological data (not even onset dates) on these cases.
For now, HPAI H5N1 appears to lack the ability to spark a human pandemic, but that happy state of affairs could literally change overnight. Unfortunately, we continue to treat HPAI as more of an economic or political problem, than a true public health concern.
A risky strategy that may work in the short term, but has the potential to end very badly.

An analysis of the sequence indicates:
This virus is genotype B3.13 clade 2.3.4.4b HPAI A(H5N1) with each individual gene segment closely related to viruses detected in recent poultry outbreaks and infected dairy cattle herds made available from USDA. Among human virus sequences, A/Colorado/109/2024 is most similar to the genome of the human case in Michigan.

Overall, the genetic analysis of the HPAI A(H5N1) virus in Colorado supports CDC's conclusion that the human health risk currently remains low.
While there is certainly some heartening news in these reports, it is also fair to say that the goalposts have recently been moved.
Had anyone dared predict last March - that in 4 months time - we'd have roughly 160 cattle herds infected with H5N1 across 13 states (an underestimate), that we'd be battling large mid-summer outbreaks of H5N1 in commercial poultry, and that we'd have chalked up 10 human cases in the U.S., they'd have been called an alarmist.
A reminder that we shouldn't confuse `increasingly commonplace' with `normal'.

A CDC doctor told them it was “just seasonal flu plus pink eye” and the local health authority said they’ll only take action if the state lab says to.

The CDC has 1 million tests for H5N1-in fact that is not that much on a population of 335 million US citizens. But better use them in time-to avoid a wide spread H2H H5N1 !

I expect we may already be in an early stage of a H5N1 pandemic. H5N1 is that widespread in mammals around the globe-mostly after eating infected birds/prey it may be impossible to see further spread into humans.
For that matter pets are a risk. Cats, dogs may get in contact with infected (dead) animals and catch the virus.
Also "droppings" (infected birds, mice, rats) are a growing risk.
Just my non-expert view...Trying to make up my opinion...
CoViD -less immunity- may give H5N1-flu more room...A risk may detecting CoViD then not testing for H5N1...

We've seen calls for expanded testing of cattle (and milk) for months, but until now the dairy farmer has largely controlled whether their cattle would be tested. Since there are negative consequences to having a herd test positive, there has been more than a little resistance to the idea.
The industry's hope, that the virus would `burn itself out' in cattle, has not come to fruition.
While it comes pretty late in the game, the testing requirement ordered in Colorado should help quantify the size of the problem in that state. Hopefully other states will now reconsider their passive approach, and order similar testing programs.
But I'm not going to hold my breath.

HHS & the FDA are finally taking the bird flu crisis more seriously & are now in crisis mode. Unfortunately, they are also taking a huge step backwards in protecting workers from bird flu.
HHS just introduced an amendment that will roll back protections so that workers can be given expired products including N95s and be forced to reuse disposable masks instead of discard them after each use. That strategy cost 1000s of HCWs their lives and will harm farm workers just as bad.

Older populations might be more protected than younger ones because of exposure to ‘matched’ strains during childhood, but an H5N1 pandemic is likely to take a major toll all the same.
Thus, people born before 1968 have tended to escape H5N1’s ravages, because they probably had their first flu infection at a time when the dominant flu virus in circulation matched H5N1. But people born after 1968 eluded the worst of H7N9, because their first encounter with flu was probably with a virus that matched it rather than H5N1. Immunity from a first infection provided 75% protection against severe disease and 80% protection against death with a matching bird-flu virus, the authors found.
If an H5N1 outbreak were to occur, this first-bout effect predicts that older people could once again be largely spared whereas younger people could be more vulnerable, Worobey says. “We should have that somewhere between the back and front of our minds,” he says.

In a follow-up to the announcement earlier this afternoon from the Colorado Health Department, we have the following statement from the CDC on the three latest confirmed H5N1 cases among poultry workers.

Confirmation of these three additional cases brings the total number of human cases of H5 bird flu reported in the United States since April 2024 to 13. Prior to 2024, the only previous human case of H5N1 bird flu in the United States was reported among a poultry worker in Colorado in April 2022. Counting that case, there have been 14 total human cases of H5 bird flu in the United States since 2022.

Let's put this in perspective....
Since 2003, there have been a total of about 880. In 21 years - that's it for the entire world.
And now, we have it in our chickens AND our cows with the USDA STILL

DJ-The only thing positive on the US H5N1 story is they at least admit they have one...Very likely other countries may have human H5N1 cases-but do not even test...
Ways people may get infected;
-raw milk from infected cattle
-raw eggs from infected poultry
-dried up droppings air borne
-infected pets
-unclear if some newer H5N1 variants may see airborne spread around the farm ?
-human H5N1 cases traveling/H2H...

DJ, Flu virus and corona virus both are RNA virus but they "spin/turn" in opposite directions...No idea if that is good or bad...
Also no idea on "what CoViD and Flu" can "do" with eachother...
-recombination ? Part of a flu virus and a corona virus form a new virustype ?
-switch genes-causing mutations ?
-"follow" eachother-co-spread...if a host is "open" for one virus it will be open for the other virus ?
H5N1 and flu can go for non-human hosts...both must be widespread in mammals around the globe...So did the virus-types interact ?
