Australia is experiencing a resurgence of mpox, with infections this year surpassing the 2022 outbreak.

So far, 152 cases have been recorded across the country, prompting alerts from state health departments.

Mpox case reported in a nonresident in Honolulu County

The Hawaiʻi Department of Health is reporting one additional case of mpox, formerly known as monkeypox, in a nonresident diagnosed in Honolulu County.

This is the fifth case diagnosed this year in Hawai‘i and brings the total number of cases reported to the state health department since June 3, 2022, to 51. Those at higher risk of mpox infection should consider being vaccinated with two doses of the JYNNEOS vaccine.​..

In this press release, Minister Baradahana explains that it is a 9-year-old girl who was consulted on July 7, 2024, at the Muramba clinic in Mugamba commune of Bururi province for generalized rashes, a cough, a fever of 40 degrees Celsius. Unfortunately, this girl died the next day, July 8, 2024. The same symptoms were observed in two other members of her family. Two other suspected cases were reported at the Kamenge military hospital and the police hospital in Bujumbura town hall and are under medical monitoring.

After learning of this information, the Ministry of Public Health and the Fight against AIDS quickly dispatched multidisciplinary teams, with the support of WHO, to conduct in-depth investigations and take samples for laboratory analysis. A total of 5 people were sampled. After analysis of the samples by the National Reference Laboratory of the INSP, she said, the results were negative for Mpox. As things stand, the investigation teams are busy carrying out differential diagnostic tests to eliminate other pathologies such as chickenpox, measles, herpes, syphilis and drug allergies.

Central African Republic: Health Ministry Declares Monkeypox Epidemic in Bangui

The Ministry of Health declared on July 28 in Bangui, the epidemic of Monkeypox, a disease also called monkeypox in the Central African capital. South Kivu: 68 monkeypox patients share 17 beds in Kavumu, warns civil society

In total, 68 monkeypox patients share 17 beds at the Kavumu Hospital Center (CH) in Kabare territory (South Kivu).

The civil society of Kavumu, which is warning about this situation, fears a large-scale spread due to lack of adequate support.

In a press release on Sunday, July 28, 2024, Justin Mulindagabo, president of the civil society of Bugorhe-Kavumu, emphasizes that this health structure requires urgent support for better care of monkeypox patients. He reports that new cases are recorded almost every day in this health zone.

"We need urgent support. Otherwise, Kavumu risks exploding with monkeypox disease. At the Kavumu Hospital, patients are lacking space. We have identified 68 patients sharing 17 beds, this is abnormal," he said.

Furthermore, he calls on the population to respect hygiene measures and to avoid prayer rooms in the event of the presence of signs of this disease. For the moment, the Miti Murhesa Health Zone has already recorded more than 100 cases of monkeypox in less than two months.

While it isn't getting the attention it deserves, for the better part of a year we've been watching the rapid spread of a new, and arguably more dangerous, Mpox clade (Ib) which was first detected in the DRC in 2023 (see WHO Reports 1st Confirmed Cluster Of Sexually Transmitted MPXV Clade 1 in the DRC).

Meanwhile the milder clade IIb virus - which began its world tour in 2022 - continues to cause sporadic outbreaks around the globe (see WHO DON Mpox - South Africa). 

Last March a study was published Eurosurveillance: Ongoing Mpox Outbreak in South Kivu Province, DRC Associated With a Novel Clade I Sub-lineage, which contained the first genomic analysis of samples from a previously unaffected region of the DRC (the city of Kamituga). 

That study revealed a novel clade I sub-linage had emerged - most likely from a zoonotic introduction - which included changes that may render current CDC tests unreliable.

In April, in Preprint: Sustained Human Outbreak of a New MPXV Clade I Lineage in Eastern Democratic Republic of the Congo, we saw a further analysis, which called for this new lineage to be named Clade Ib, and warned of its potential to spread globally. 

While that hasn't happened yet, the Mpox epidemic in the DRC continues to worsen, and neighboring nations are on heightened alert.  In recent weeks Mpox cases have been reported in Burundi, and given limited surveillance, other cases may be flying under the radar.

The caveat of course is that up until mid-May of 2022, the risk of Mpox Clade II spreading internationally was considered equally low The Mpox virus was only belatedly discovered to be spreading in multiple non-endemic nations, leading to the declaration of a PHEIC (Public Health Emergency of International Concern) the following July. 

As we've seen with COVID, H5N1, and other infectious disease reporting around the globe, surveillance and reporting on Mpox is often limited, or sometimes missing entirely.  The WHO described this situation in their last monthly Mpox update #34:

WHO continues to encourage all countries to ensure that mpox is a notifiable disease and to report mpox cases, including reporting when no cases have been detected (known as ‘zero-reporting’, as outlined in the Standing Recommendations on mpox issued by the WHO Director General).

This report does not highlight non-reporting countries. Therefore, it should be noted that an absence of reported cases from a country may be due to the country not reporting, rather than having no cases. Reporting to WHO has been declining, therefore, the decline in reported cases should be interpreted with caution.

A reminder that an absence of evidence is not necessarily evidence of absence.

Mpox is a zoonotic disease, which means it can spread between animals and people. The exact source of mpox in nature is not known, but it is believed that small mammals, such as rope and sun squirrels, giant-pouched rats, and African dormice may carry the virus in parts of West and Central Africa.

DJ..why do we then NOT see any testing in mammals for Mpox ?

The Ivorian Ministry of Health announced that it had recorded two "non-fatal" cases of Mpox, a virus long called "monkeypox", the first reported in this West African country since the resurgence of the epidemic in 2022.

" The first case is a 46-year-old patient (...) who consulted on July 1 for fever, headaches and skin rashes. The second case, aged 20 (...) presented on July 14 with skin and oral mucosal rashes ," said the ministry, which added that the diagnosis was made after analysis at the Pasteur Institutes in Abidjan and Dakar. A spokesperson for the Ministry of Health told AFP that the lives of the two patients were not in danger and that they were being monitored.

The disease is characterized in particular by skin rashes on the genitals or in the mouth, and may be accompanied by fever, sore throat or pain in the lymph nodes. Mpox was first discovered in humans in 1970 in what is now the DRC (formerly Zaire), with the spread of the Clade I subtype, mainly limited since then to countries in western and central Africa, with patients generally being contaminated by infected animals.

In May 2022, Mpox virus infections occurred worldwide, via another subtype of the virus, mainly affecting gay and bisexual men and had caused some 140 deaths out of about 90,000 cases in 111 countries. But since last September, a new, more deadly strain has been spreading in the DRC with nearly 450 deaths out of 11,000 cases.

MPOX " remains a threat to global health ," WHO chief Tedros Adhanom Ghebreyesus said in early July, with the organization saying that cases had been reported in 26 countries in one month.

According to information confirmed by the Ministry of Health and the Pasteur Institute in Bangui, nine cases have been recorded in the capital in one week.

Landry, a resident of the 6th arrondissement, one of the hotbeds of this disease, is on his guard: " It's scary because people are isolated in the hospital. I learned that this disease is very contagious and that we must avoid contact with dead animals. I limit my movements, because it is also transmitted between humans ," he says.

While the symptoms are unknown to a significant number of Central Africans, the health department is increasing awareness campaigns to block the spread of this disease. 

" Monkeypox is a disease that manifests itself like smallpox, with rashes on the body and pus loaded with viruses, with fever. Secondly, there is also an infection of the lymph nodes. This results in an increase in their volume. This disease can last 14 days and can lead to death in 10 % to 11 % of those affected. It is therefore very serious ," explains Dr. Pierre Somsé, Minister of Health.

Between January and July 2024, 166 suspected cases were reported throughout the Central African territory. According to the Pasteur Institute in Bangui, 35 cases are confirmed for zero deaths.

Earlier this week, in ECDC Risk Statement On Mpox As Cases Continue to Spread In Africa, we looked at the increasing spread of (both clade I & clade II) Mpox across endemic regions of central Africa (see map above), and feared incursions into neighboring countries. 

In the past few days both Kenya in east Africa, and Côte d'Ivoire in west Africa, have reported cases, raising further concerns that the virus is gaining territory. 

The Central African Republic (CAR) has also declared an outbreak after cases - which are normally reported from more rural regions - have been reported in their capital; Bangui. 

Complicating matters even further, we've seen reports for the better part of a year on a new variant of the virus - dubbed clade Ib - which is spreading in the DRC (see Preprint: Sustained Human Outbreak of a New MPXV Clade I Lineage in Eastern Democratic Republic of the Congo).

While this new clade has not been confirmed outside of central Africa, it appears to be more easily transmissible and more severe, sparking a number of recent risk assessments.

Since the eradication of smallpox in the 1970s, there is a general feeling that poxviruses are a thing of the past, a relic of the 20th century. 

But as our collective immunity from the smallpox vaccine - which was discontinued in the late 1970s - dwindles, there are increased opportunities for pox viruses to make a comeback. 

The Mpox virus is continually evolving, and we now have at least 4 clades of Mpox in circulation (I, Ib, II, IIb), with Clade I and Ib considered the most severe. 

As they spread from host-to-host, additional evolutionary changes seem likely (see Evolution of monkeypox virus from 2017 to 2022: In the light of point mutations).

All of which means the next Mpox virus to begin a world tour may not prove as mild as was the IIb clade, which emerged in 2022.
